Checklist for ordering by mail or fax

• Consider ordering online and paying with a credit card.
• Add items to shopping cart.
• View shopping cart and select shipping method and gift packaging options, then click Update Totals.
• Come back to this page (bookmark it now) and request your order form. All items in your shopping cart will be included in the order form.
• (optional) If you'll be sending your payment information by postal mail, email your completed order form so we can reserve your chosen items. We'll email you a confirmation via email when we've done this.
• Mail or fax your order form and payment to the address/fax number shown on the order form.
